ADMISSION PROCEDURE
The admission procedure for the school is purely based on an Entrance Test for the classes of VI and
based on an open Lottery System for the class I.
The parents/guardians have to collect the admission form from the school and submit the filled up
form within the dates provide by the school. A tentative date is given for the entrance test as well as
for the draw of lots. The draw of lots is done in presence of all the members of SMDC and Higher
Authority.
Seat Capacity Details –
Class I –Two Sections – 40 x 2 = 80 Students
Class VI – Two Sections – 40 x 2 = 80 Students
Minimum 50% of seats in both Class I & VI are to be reserved for girls students.
Age limit for the admission

DOCUMENTS NEEDED TO SUBMIT WITH THE APPLICATION
a. Photo copy of Birth Certificate, Cast Certificate
b. One extra stamp size photograph of the student staple along with in this form
c. Address proof certificate of the parents (GP certificate/Voter ID Card /Driving License etc.)
d. Duly filled up Medical Emergency form stating details regarding allergies, asthma, diabetes etc. Medication and first aid needed at the time of emergency
Original Certificate Required for Verification Purpose
a. Original Birth Certificate, Cast Certificate (if applicable),
b. Single Girl Child Certificate from a government Medical Officer (if applicable)
c. Physically Challenged Certificate (if applicable) must be produce along with this form for verification purpose while submitting the form.
